If you currently live or are traveling in another state and need to submit fingerprints electronically to the Bureau of Criminal Identification (BCI) for any of the reasons listed below then we can it done for you. Our card scan service can convert a “hard” fingerprint card into an electronic fingerprint record so you don’t have to travel to Utah. No other state can submit fingerprints electronically to BCI. After we have received all required items we submit them electronically to BCI the same day which allows your background check process to begin.

What You Need

1. Copy Of Your VALID Government-Issued Identification
Your identification cannot be expired and must contain a photo, physical description, signature, date of birth, etc.

What Is Acceptable Identification?

Government-issued picture identification that is valid (not expired) with the individual’s name, date of birth, signature, or fingerprint is required to ensure proper identification unless otherwise prohibited by law. The ID must be legible and intact.
The exceptions to having a signature or fingerprint are government-issued Military IDs, Passport Cards, Permanent Resident Alien Cards (Green Cards), and Employment Authorization Documents (EAD) (these contain no visible signature).

2. Company/Agency Specific Form

In order for us to transmit fingerprints electronically to the Utah Bureau of Criminal Identification (BCI), you must provide any and all authorization forms from the agency that is requesting your fingerprints to your appointment and it must include the billing code (B####).

We must retain the original form for auditing purposes. If your employer requires a copy of the live scan direct submission form please let us know.

3. Fingerprint Card
Obtain a completed (FD-258) fingerprint card.

You can obtain the fingerprint card from your local law enforcement agency, such as a sheriff’s office, police department, or other authorized private agency in your state. If you are having difficulty obtaining an FD-258 fingerprint card, please contact us.

All of the following areas must be properly completed on the FD-258 fingerprint card:

  • NAM – Your full legal name in the following format: LAST NAME, FIRST NAME, MIDDLE NAME (full middle name not just the initial unless applicable)
  • SIGNATURE OF PERSON FINGERPRINTED – Your signature
  • RESIDENCE OF PERSON FINGERPRINTED – Your full residence address including any apartment or unit number
  • EMPLOYER AND ADDRESS – not needed
  • REASON FINGERPRINTED – The reason you need to be fingerprinted
  • DATE – The date your fingerprints were taken. Your fingerprint card must be received by us within one year of this date
  • SIGNATURE OF OFFICIAL TAKING FINGERPRINTS – The individual signature who completed your fingerprints
  • ALIASES “AKA” – Any other names used, maiden names, etc.
  • CITIZENSHIP “CTZ” – Your citizenship: USA or other acceptable country code
  • YOUR NO. “OCA” – Only if applicable
  • UNIVERSAL CONTROL NO. “UCN” – not needed
  • ARMED FORCES NO. “MNU” – not needed
  • SOCIAL SECURITY NO. “SOC” – Your full nine (9) digit Social Security Number (no dashes)
  • MISCELLANEOUS NO. “MNU” – not needed

4. Signed Consent Form
Complete and sign our consent form. We must have your written permission to submit your fingerprints to BCI. You will be able to attach your identification and Live Scan Request Form.

Fingerprint requirements for the Utah State Board of Education (USBE) Educator Licensing

The Utah Board of Education is using a new system called Utah Schools Information Management System (USIMS).

You Will Need:

You must have the Live Scan Fingerprint Processing form or the Live Scan Fingerprinting Authorization Form from Child Care Licensing (CCL), Utah Department of Health.

The Office of Background Processing has recently changed its livescan fingerprinting authorization form as well as how that form is obtained. The form will be automatically generated through the Child Care Licensing portal by the childcare provider. Demographic information will be automatically generated on the form.

  • We cannot accept any other form for childcare purposes.
  • We cannot accept any forms that have been handwritten.
  • If you do not have the correct form with the generated demographic information, we cannot fingerprint you.
  • We CANNOT provide this form for you.

It is very important that you follow the correct process of obtaining this Livescan form in order for the Child Care Licensing to complete the clearance. If you have any questions please contact the Office of Background Processing at (801) 273-2821 or email [email protected].

Do not make a fingerprinting appointment unless you are within your license renewal window, 45 days prior to expiration for a Real Estate license and 3 months prior to expiration for an Appraisal license.

You must have the Authorization for Direct Electronic Submission form from the Utah Division of Real Estate. To obtain this form go to the Division of Real Estate website.

  • Select “RAPBACK SERVICE”
  • On the “Welcome” page, select “CLICK HERE TO BEGIN THE PROCESS”
  • Enter your license number and the last 4 digits of your social security number and log in.
  • Select the button for authorization consent (Utah Consent), select agree, and click “Next Step”.
  • Click on the red button “DOWNLOAD FINGERPRINT AUTHORIZATION FORM”.
  • Print the form and bring it with you to the appointment.

We cannot accept forms where the OCA (license number), applicant name, date of birth (DOB), or license number are handwritten. The form data must be computer generated except for the social security number (SSN), fingerprint date, and signature.

Fingerprint submission is a one-time requirement.

After successfully completing a state-approved course (Training Courses), completed the application at https://emslicense.utah.gov within 30 days course start date, and pay the required fee(s) then you can submit the required Division of Licensing and Background Checks (DACS) fingerprints for criminal background check (one time requirement).

EMS Students must bring a LiveScan Authorization form, which is emailed after the fee(s) have been paid at: https://bemsp.utah.gov. You can access this form in the Documents area on the left-hand side of your account. “My Account > Documents”, and the file will be saved as DACS Form.

 

  1. Check the form for any inaccuracies or ‘unknown’ information, and the expiration date on the bottom of the form. If there are any errors please email [email protected] to be corrected.
  2. Print out your LiveScan Authorization form. It is recommended to use a computer to print your form
  3. You must bring this form to your appointment for us to complete your fingerprints.
  4. We will input the OAC/Agency Code. These codes will report your background directly to the Bureau, there is no need to send the Bureau of Emergency Medical Services and Preparedness (BEMSP) the signed form. Forms will expire in 90 days.

Criminal history background checks are required for all owners and persons employed to act in a supervisory or managerial capacity for a package agency, licensee, or permittee.

New Department of Alcoholic Beverage Services (DABS) license applicants must submit fingerprints either prior to submitting their application or with their application, so please plan ahead for fingerprinting services so you can meet your deadline before the 10th of the month. Background checks for each applicable person must include fingerprints, either through a live scan service or a completed FBI fingerprint card, and a signed Informed Consent & Waiver form for each individual fingerprinted.

You must bring a completed live scan authorization form. There are 4 pages, the last page is the Live Scan Authorization Form. This Authorization is required to electronically transmit your fingerprints. Download Form
